Below are the Kent and Medway documents that support the governance process for reviewing and managing changes to the Joint Medicines Formulary. All forms are currently in a pilot testing phase, and feedback is welcome.

1. New Medicine Formulary Application Form
For all applications for new medicines to be recommended for inclusion in the Kent and Medway Joint Formulary (with the exception of medicines with a positive NICE TA)

2. NICE Technology Appraisal (TA) Governance Form 
For all requests for medicines with a positive NICE TA to be recommended for inclusion in the Kent and Medway Joint Formulary.

3. Formulary Amendment Request Form
For changes to existing formulary entries, such as changes to formulary status, deletions, substitutions or temporary additions (e.g. due to shortages) as well as the inclusion of new formulations or new indications for medicines already listed.

4. SBAR Form (Situation, Background, Assessment, Recommendation)
A structured document for submitting new or amended clinical pathways, prescribing guidance, or options appraisals

5. Single Patient Request Form

For Trust use only: For clinicians to request single-patient or one-off use of a non-formulary medicine within Kent and Medway Trusts.

Submitting an application

Please send completed request forms to your formulary pharmacist within your local secondary care trust, or to the Medicines Optimisation team in primary care.

Applications will be reviewed by the Kent & Medway Integrated Medicines Optimisation and Prescribing Group (IMaP), with recommendations submitted to the Integrated Medicines Optimisation Committee (IMOC) for approval.

Single Patient Request Forms will be reviewed by the Trusts Drugs and Therapeutics (or equivalent) meeting.
